If your mom enters long term nursing care and is on Medicaid, does the nursing home get to take your mom's monthly social security check?

Yes, Medicaid only covers the medical portion of her care, and her SS covers the custodial (room and board) portion. My MIL was in LTC for 7 years and our state allows her to keep $90 per month. She barely used even that much because almost all her needs are taken care of already.

Yes minus a small personal needs allowance that is determined by state. Usually ranges from $40 to $125. The check pays mother’s share of cost for the nursing home. There is no money left to maintain house, take care of family or pay bills outside facility.
