I think we need more information, Nancy. I think also that there comes a time when one on one 24/7 care doesn't work and quite honestly what is required is a team of several shifts of several caretakers each. That means extended care facility. Only you can know if you are approaching that time. When things get this bad you are looking at end stage disease; the problem with THAT is that the end stage can go on endlessly at times.
Give your parent a choice. They either have their brief changed regularly or go into AL or a nursing home where it'll be done for them, whether they like it or not. Some things are not optional.
