They are probably: 1) Medical Representative (a HIPAA form filled out by the principal at each doctor's office, naming a representative; has to be filled out annually) 2) Medical Power of Attorney 3) Advance Healthcare Directive 4) POLST (Physician Orders for Life-Sustaining Treatment) a medical order that communicates a patient's end-of-life care preferences to healthcare providers. It gives more specific instructions about their treatment than other advance directives.
What do you think the 3 dicuments are. Three that Mom drew up in a lawyers office were her Will, Financial POA and Medical POA also called an Advance Care directive. In that POA/directive it usually lists what the principle wants or doesn't want concerning their health.

I honestly could not imagine what you are referencing, yet when I get Mr. Google on it, asking what the three legal documents required to manage care this is what I come up with:

If these are the documents you are referencing I would ask your senior's doctor for them. They should be easily able to either supply them to you (Kaiser has a GREAT advance directive) or show you their favorites for their state online.
