I was always given a choice of Rehabs when Rehab was recommended. Its wherever a bed is open. I see no problem in you finding a facility closer to home unless the Rehab he is in specializes in what he needs. You probably will be responsible for transportation to a new Rehab. They may not discharge without it.

I doubt if you can. A rehab is basically decided on where the doctor orders it. I am afraid you are down to phone calls for a while. I wish you good luck and hope that he does very well.
The Doctor can Order in Home PT - Physical therapists will come to the House . He can also get you a VNA Nurse and a CNA referral who can help with bathing .
Whether the doctor prescribes in-home therapy or therapy at a rehab facilitytypically depends on the patient's medical needs. Typically, a person who is sent to rehab needs comprehensive rehab, between 4-6 hours/day, and may also need some assistance with ADLs. In-home rehab is usually only a few times/week for 1-2 hr at most and is typically prescribed either after a person returns from a rehab facility but prior to the person's being physically able to go to an outpatient therapy facility, or immediately after hospital discharge when comprehensive rehab is not needed but the patient is not yet physically able to go to an outpatient therapy facility. That latter situation, for example, may occur after a knee replacement.
If he is cognizant and making his own decisions he can look for a Rehab that is closer to you if that is what he wants. As long as the facility has a bed and will accept his insurance. The one facility can arrange transport to the new one.

Please do the research and find the BEST rehab facility. That way he can get rehabilitated and come home.
