If so, how often would pay be?

Frequency of payment should be completely unrelated.
Paid per patient? Legal? I don't think so!

People are generally paid by the hour or a salary is calculated based on the work provided just like any other job. Some people do make weird arrangements that often don't comply with labour laws, but I would never recommend going that route.
