Anyone ever completed a comprehensive plan of care to submit to Medicare???
Medicare says they need this immediately to help cover costs, but each doctor we speak with says we (as the family) need to come up with a plan of care for them to sign. I guess I'm looking for a template??? Please help.
I am curious because I have never heard of this before. Why would family be responsible for a "care plan". What are you trying to get, help in the home? Surprised that Medicare didn't tell you how to go about this. When you get the form, you may want to call Office of Aging to see if they can help with filling it out if it becomes confusing.
