Is this for you or a relative?  If any chance of a diagnosis of dementia, get POA in place first

Need more info. Is this for you? Is this for an elder in your care? Do you have POA so that medical info can be shared with you? The best thing you can have is a list of happenings that make you question mental capacity, whether your own or the person you are speaking of. The doctor will do minimal exam, knows the questions to ask, will refer to further eval for neuro testing and scan if indicated. More detail on the question might help further answers.

If you have Medicare, a cognitive test is suppose to be done with your yearly physical. If you are under the age of 65 you can notify them of your concern when you schedule the app't. Keep a log of examples you have experienced. After your app't. if they feel it is a concern they will refer you to the appropriate specialist.
