Does anyone have a sample agreement for hiring an independent caregiver to assist two hours a day for elderly parents in their home?

Services will be for a limited time period so payments will not exceed the $2200 limit for social security. Does home owners insurance provide coverage for any damages? Thanks for any advice or other pitfalls we need to think about.
