My grandmother has swelling on neck untill her young age and she nowadays diffficult of breathing. Any suggestions?

It was long time having lump on neck at least 40 years back but she deosnt have any pain or complain along, but now she is feeling difficult of breathing and her upper chest has dilated veins and arteries are vissible and lump is enlarged enough
