Gathering information to prepare for mom's future care.

Mom moved in a year ago. She is 82 and has diabetes, RA and dementia. I give her meds, insulin and leave breakfast, lunch and snacks before going to work. A day will come when she will need full time care. Mom does not qualify for medical yet she doesn't have enough income for assisted living. She doesn't have Medicare cause she has Kaiser through Federal (dad's retirement). I appreciate any recommendations/ advice on how to prepare for future care for my mom. We live in California.
