What do I do when an 89-year-old is showing signs of dementia?
She recently has foot problems and is going to physical therapy, she has me bring her. Running over light post, mailbox, hitting garage, has dents all over the vehicle. She told son she will tell him when she can no longer drive.

If he won't report her to DMV, then usually, there is a form that you can fill out to report an unsafe driver.
