How to execute the legal POA and durable medical POA?

My mom needs NH she doesn't agree. She falls lots. Recently breaking her nose. She needs more care than I can provide as I have a bad ankle and leg I've had for 16 years and it is worsening. How does this work? Can't afford attorney am on very low income. Thanks for advice.
