I need a copy of Five Wishes. Can I come to the office and pick one up?

Mysticalrose, in Michigan it is called a POLST form. You get one from your doctor's office, since the MD has to sign the form also.

Mysticalrose, I never heard of the Five Wishes so I looked it up on the Internet. Apparently it is some type of Living Will for a person to fill out. There are websites that offer this product.

Personally, I would consult an Elder Law Attorney and have him/her prepare a Living Will. I am not a big fan of the do-it-yourself when it comes to legal documents. One missed word or one misplaced word can create problems.
