How do you gain guardianship of a parent who is unwilling?

My mother has had a stroke. She receives a stipend from the military but is often broke within the first week of pay. She is not willing to keep up with personal hygiene and has verbalized suicide multiple times. I'm truly concerned for her well being but don't know what i can do to help.
