Where to find a social worker with whom I can consult on medical and long term care problems?

I have 70 yars old wife who is diagnosed a parkinson's disease.she lost her control of mobility. I am her husband of 80 years old. i can hardly of any help to her. I need somebody who can consult with about long-term care program.
