I agree. A social worker at social services should be able to take your application and depending on what you qualify for, provide you with the information of what is available. Most cities have income based housing. I would make sure to ask about that.
You state that you are disabled. I learned from my research that those who are disabled often qualify for more in the way of benefits than those who are not disabled. Do you need assistance with your daily care? If a doctor can state that you need assistance with things like meals, medication, bathing, etc. you might qualify for assisted living. I would ask the social worker about that as well. It probably depends on the state you are located.
My loved one was disabled due to arthritis, but needed admission to assisted living due to her dementia and the fact she could not take care of daily needs anymore. I wish you well.
