Who can start the process for caregiver services?

If someone has a social worker come to do an assessment without the client.

When my grandma got hurt, the doctor made a referral and we were given the name of the local state agency. When we did that, we received a social worker's name and she met with us, helped us to find an agency that worked with us and a nurse came out and set up the services. Once we contacted the agency we chose (the social worker offered and should have made the call but she took forever and a day so we made the call and started the process), they sent out a nurse to evaluate what was necessary and it took off from there. The nurse of the agency handled all the necessary care which included caregivers, therapists and her own weekly care.
