Do you have a list of visiting physicians, not associated with the VPA?

My Mom has Alzheimer's and needs to see a phyiscian. She does have mobility but refuses to go downstairs in our home (the last time was November 2013) I need a visiting physician and in home heath services. She is in very good physical health. She takes only two meds, lisnopril and ranitidine. Help. She has APWU Cigna PPO insurace and also has a LTC policy in place. We used the LTC for adult day care. However, because I am employee I can care for her. I took an Alzheimer's Association Savvy Caregiver training class in 2009. She has no medical emergency at this time, however I need to make sure that she has a Physician.
