A regional Elder Services agency (like Cape Cod & Islands), which focuses on health rather than recreational issues, can help with support now, as well as in the future as your father's needs increase: they work with the Visiting Nurses Association (home aids, therapists...), social workers, provide respite care, support groups, and advice about home accommodations.

I guess the best advice I can give you is to remember to take care of yourself. Don't feel guilty taking time for yourself. Maybe go to the local senior center or call and see what is offered to help you both or call your local town hall and ask the council on aging for some direction.
