Has anyone known of skilled nursing facilities to encourage mostly independent patients to stay definitely and to create division between patient and family?

Is there a standard in California for Medi-Cal patients to have some sort of reasonable discharge plan? Also, is there a standard in California for boundaries between patients and staff in nursing homes? I understand that Medi-Cal will not pay for assisted living in California. Is that true?
