Is my Mom's house at risk?

My mom and dad got legally separated 15 years ago the court gave her the house but she never took his name off the house she has her paperwork from court stating the house is hers but now my father had a stroke and need to be in a nursing home will her house be in any danger of being taken

Probably good to consult an attorney. The document stating that the house is your mom's is probably what's most important, but you may need some direction in how best to make it clear and remove your dad's name from the title. You could go to your local title company, present the document and see what they can do to have your dad removed from the title. The date of the document would seem to be the most important issue and it should be noted in changing title.
