How can I move my grandmother who is living on a hospital bed with a catheter from ARK to VA?

My 94-year-old Grandmother has severe dementia. I have a durable Power of Attorney and a doctors note stating she is incapable of taking care of her self. she is living on a hospital bed with a catheter. Her money is running out. How can I move her from ARK to VA?
