Legal Issue

I am trying to find out about an issue with a DNR client. I was told by GT Financial that since my client is a DNR that his wife and I would need to get the document signed by my client from his doctor and his wife and I were to go to a notary and get it stamped and signed. She refuses to do this. I am legally responsible if I do not use CPR since that is his wish?

I meant to say, I am legally responsible if I revive him? There is a DNR document from Great Lakes in his folder which she did not reveal to me until this Thursday. I did call his case manager several times to ask this question and she failed to respond back. I talked to a nurse from Great Lakes she said since I do not work for Great Lakes I need my own signed document. Does anyone know the answer to this question?
