I have to first ask...why are you allowing your husband to wear his boxers when he is incontinent from both ends? That's crazy! Throw the darn things away and only allow him to wear the Depends, and then like already mentioned have him wear the anti-strip clothing available on Amazon. And of course if this is now getting to be too much for you to handle at home, it may be time to look into having your husband placed in the appropriate facility.
Yes, almost certainly at this point your husband needs nursing home care or memory care where there are several shifts of workers and several workers on each shift. This isn't sustainable when late stage dementia robs the one you love of all power to reason, do self care, or help care for self. I am so sorry.
Question is - is he taking them off because he feels they are soiled? we use disposable underwear and inside pop an incontinence lad so it’s easier to change maybe you need to establish a routine with him and keep telling him we will change at 8 and 12 and 4 and 8pm sort of thing so you don’t need to worry and remind him of the tones maybe on paper put up somewhere just a suggestion but I’ve not experience if Alzheimer’s so just a suggestion if it can help good luck
There are clothing pieces that are more difficult to remove. Anti Strip clothing You also need to do a more scheduled bathroom break. Every 2 hours get him up and into the bathroom. Time the trip to the bathroom with a task he can do after. Helping with the laundry, peeling potatoes, putting away some dishes, a snack anything that will be a reason for him to get up. And if you do go with the anti strip clothing you WILL have to take him to the bathroom so you can help him get undressed. Bathroom issues is one of the breaking points for many people deciding to place a loved one in Memory Care.

I mean duct tape the diaper so he can’t get it off!!
