I need a personal case manager for my mother in Louisville, KY.
Mom lives at the Enclave, but was recently hospitalized for two weeks. They moved her to a horrifying “rehab” center yesterday. She has to go to rehab before going back to the Enclave. I live out of state. We need help.
Do go to the internet and type in Nurse Care Manager and the area for which you wish to hire him/her. We are just a Forum of caregivers, so don't think we can help. Also try to go to Nurse.org/care manager. Good luck. Might I ask how, from out of town you are assessing all this at present? Is there any way you can now travel to mom to personally assess what's up?
