Can I claim a client I caregive for and we're living together but I don't receive any pay for being his caregiver?

I was asked to live with Tommy to be his caregiver. It's been 14 years now. I don't receive any pay because he's on Medicare. I was wondering if I could claim Tommy on my income tax?
