Mom has dementia and I am her activated healthcare POA. Can she sign paperwork to change estate executor and sign a personal care agreement?

No, if her Dementia is pretty far along. She is not competent to make these decisions. The Executor is not ground in stone. When Mom passes, the one she assigned can step down and Probate assign someone else, usually an Administrator.

I would ask her doctor to evaluate and see where Mom stands cognitive wise. Then have him write a letter saying that she is confident enough to reassign an Executor and make a care agreement. Then go to her lawyer with the letter. Be on the up and up or it can come back to bite you.
