Client is lying about abuse. Any advice?

Client is in a beautiful and loving home. They are all more than well cared for. Situation is like a big family. Client seems to make terrible accusations that may lose caregiver's license.

Or, is there any obvious reason - such as dementia or another form of mental illness - why the client would be behaving this way?

If you are in charge of this home and your client is making unfounded accusations of abuse, you'd better seek advice on how to handle it from APS. There should be a formal procedure in place for resolving issues and complaints, including serious complaints, anyway.

All abuse accusations would have to be investigated. Is there evidence to back up the accusations?
