My dad with dementia has become combative (hitting, kicking). He has been taking Seroquel 25mg. What is another option besides Ativan?

Ativan seems to make him more agitated.

SOOOOOOOOOOOOOO many options. A daily antidepressant that also deals with anxiety (which often presents as being combative), a benzodiazepine (Ativan is one) there are many! Seroquel is very sedating, used for sleeping aid, usually.

Talk to the psych dr in charge. They can work wonders with psych meds to level out the person's state of mind. The dementia makes everything harder, but a good geriatric psych dr could handle this.

And yes, some people react "opposite" to a drug. Klonipin is a good benzo, slower to act, but longer acting. Seriously, so many options, so don't give up hope.

Hope you're not caring for him at home. Hide the sharps f you are!!
