Fmac, unfortunately you cannot transfer Colorado Medicaid to Minnesota. I assume this is for your father [as per your profile], he would need to apply and qualify for Minnesota separately.
The reason for this is that each State handles their own Medicaid, and have their own programs. Programs available in Colorado may not be available in Minnesota, or you might find better programs.
As soon as your Dad crosses over the Colorado State line, his Medicaid stops, but if he has Medicare, it is in full force nationwide if you find you need care on route to Minnesota.
