How do I get a replacement Social Security card for my husband who has Alzheimer's?

He does not have an Identification card? He is in a nursing home

My father's been a a nursing home for awhile. He has dementia. We've had some problems with the care their. So I'm bringing my father home. I have been made his repressive payee, but now I found out from the buisness office of the nursing home, they have no Id, no picture Id, no soc sec card. No Birth certificate. They just have his soc sec #. How do I go about getting his soc sec card?

I was able to get my Dad a replacement Social Security card and a replacement Medicare card as both were so very worn out, and falling apart. I was able to do this on-line. https://www.ssa.gov/ssnumber/

You can go online to socialsecuritydotgov and request a replacement card.

The "representative payee" can get this done. Usually that is the Nursing Home.
