If I hired a carer, can I deduct wages and tax?

I am 72 collect social security. I work and my wife has MS. She can no longer care for self and we have no long term care ins.

Yes this is deductible as long as you have a letter from the MD certifying the need for care, and you keep a proper payroll record.
