You might want to ask her oncologist if there's a social worker associated with the infusion center or hospital, and contact that person for suggestions. Another thought is to contact Gilda's Club; they don't provide a referral service but might have suggestions on alternate living.

Were you thinking of assisted living? skilled nursing facility? or something more along the line of hospice? I think the latter might be the most appropriate at this stage of her life.

Have you considered a hospice facility? A friend of mine spent his last days at a hospice facility and it was a lovely and peaceful place.
