If you are caregiver to this person, I would try therapeutic fibbing and say that the doctor's office called to say that he needs a physical due to...something, insurance renewal, Obamacare, pneumonia vaccine. Send the doctor a note beforehand outlining the symptoms you are observing.

Nagging the patient will only make him dig his heals in about not going to the doctor.
