How do I get insurance surrender values when Mom is unable to give consent?

Medi-cal wants cash surrender values for whole life ins policies, but Mom has dementia and is unable to write her name or give verbal ok to the insurance co to give me the necessary info. How can I get this info from the insurance co? I do not have power of attorney.
