How can my grandmother or I become the appointed guardian for my grandfather?

My grandfather suffers from dementia. I am the primary caregiver for both my grandfather and grandmother. I have both medical and durable power of attorney for both of them. How can I or my grandmother become my grandfather's legal guardian?
