Many areas have Volunteer groups that will provide transportation. The one in the town where I am you have to "register" and if you need a ride they would like 24-48 hours notice. Some areas that provide public transportation will do a pick-up with an accessible van or bus for doctor appointments and trips to the store. Again the ride has to be pre-arranged. This is often free or reduced fare for seniors and disabled. Then there is the simple calling a cab.
You do not say if this is a "medical transport" where a wheelchair or possibly even a gurney is needed to transport the person. If a medi-van is needed there are plenty of companies that do that check with insurance company to see if it is a covered cost.
Also, check on Facebook. Recently I have seen two or three posts advertising Uber or Lyft-like services. And, check with local churches or senior centers as they often have volunteers who will provide rides.
