My elderly father has dementia and cannot remember the name of his long term care insurance company, how can I find this information?

Kathleen, usually we have to put on our detective hat and start going through our parent's file draws, desks, bookcases, closets, and look for the long-term-care insurance policy.

Maybe try looking at his bank statements if he'll let you. They have to have been charging him annually/quarterly/monthly. At least if you have a name to go off of and call with his identifying information you may be able to track down more policy information.
