Elderly relative needs transportation to doctors offices.

I have an aging relative who lives alone. It's getting difficult for him to drive, and he needs to see his doctors often. Taxis are way too expensive, and there are no buses near him. Are their any senior transportation services that could help? He lives in Suffolk County Long Island NY.
