If complicated go to a lawyer as freqflyer suggested. If straight forward and simple Office Depot and staples has forms but they need to all be witnessed and signed in front of a notary public.

You can always go to a "consultation" withlaw office, sometimes they offer for free, and an honest lawyer would tell you if you need specialized treatment of the documents. But no, not every one needs a lawyer to do these documents. Some states actually put them on their State website because they know not every one can afford a lawyer! Check the website for your state and follow their directions.
