Where do I turn for help?

My mother-n-law is hook on pain pills, her 3rd husband is an alcoholic.
She had knee replacement about 3 years ago and will not get out of her chair, she messes on herself, and they have dogs that do not go outside, the smell in the house makes you want to vomit. We have done up to 18 full loads of soiled laundry that was sitting in the bed room and basement.
