I have to be blunt and suggest that if you're uncertain where to request medical records, you aren't prepared for any kind of medical malpractice action or suit.

It's not my intent to offend, but your post strikes me as very peculiar for people allegedly involved in obtaining client records. And if you are attorneys, this is one of the basics: learning HOW and WHERE to obtain the data and records that are required to PROPERLY represent a client are legal basics.
