Contact the VA and see what community services are available. Ask to talk to the social service department.

Use an online search engine to determine if there are other VA facilities in your area, then try them out.   They may be farther away, so that might be a concern.

You mentioned long term care; are you in a VA care facility, or are you referring to hospital and medical visits?

Your County should have a VA department.

Problem with VA homes are they are few and far apart. I was asked why I didn't I place my Mom in one. Because it was 45 min away traveling back roads in another County. I wanted her closer.

If you truly have a problem with the care that is being given to the Veteran contact the Patient Advocate. There are also Social Workers that you can talk to.
Each facility is different and the caregivers are different. I know several people that have had spouses that received very good care at a VA facility.
There are some older facilities that have problems and I agree that they should be addressed. Our Veterans deserve far better and it is the least that the Government can do after the sacrifices that our Veterans have made.
If there truly is a problem and you get no where with the Patient Advocate or Social Worker going to your local Senator, Congressman then the media would be in order. By media I include Social, Print, Radio and TV
