The doctor may need help in completing this form, since it asks what the patient needs assistance with. He may not know the answers without input from the patient's family. For example, what are bowel and bladder needs, feeding needs, bathing needs, medication needs, etc. If she is already in a facility, that place get give the information to the doctor, he can complete it and sign it. If the patient is home bound, the doctor can get the information from the patient and her family.
