My mom has to be watched like a toddler. She eats well what is prepared for her. She needs full assistance with showering, dressing, toileting, etc. I’m wondering if this the sixth stage and what to expect next.

So, I'm not sure it matters what "stage" your mom is in. What matters is what level of care she needs and where it can be provided best.

Have you asked for a "needs assessment" by your local Area Agency on Aging? Or through mom's doctor? Us "kids" can guess a lot, but a professional can ascertain what the elder's care needs actually are.

I'd encourage you to be there for the assessment. Many elders, understandable, overstate their capabilities. Isn't that what we get taught to do in job interviews?

"Yes, I can cook myself a meal".

But does she remember to turn off the stove?

"Yes, I can shower".

But does she, or is she fearful of falling and thus doesn't shower for weeks at a time.

You get the idea.
