My Mom has end stage heart failure and she just wrings her hands all the time, even when seeming to sleep.

IMO it's a sensory issue. It's just a calming feeling.

I've never heard of that. Are her hands cold? Could cotton gloves help warm them? Are they dry, and need lotion? I bet it's kind of distressing for you to see that and wonder if she's afraid or in pain. Can you ask her when she's awake?
