Late stage of dementia MIL takes diapers off middle of the night and does #1 and #2. She is taken to use the toilet every 4 hours and before going to bed. Any advice?

We found the onesies to be a Godsend in our house. And there are many different styles to try. We have had really good results with the "tank top" style. We tell her it's her undershirt and she seems fine with that idea.

There are adult "onesies" that you could get.
This may sound odd but...try putting the tab brief (personally I do not like the term "diaper" for an adult product) on backwards so the tape is at the back so it takes a bit more effort to peel it off.
If she is pulling the brief down or tearing it off if you can't find the "onsie" PJ's with a Velcro (hook and loop strip) or snap that you can put on so that the top and bottom will stay connected.

Grandma, I'm with you...there are enough indignities in decline, and telling seniors they wear a "diaper" just adds to it.
