Contact Kalamazoo and Grand Rapids public transit agencies and inquire about door-to-door transportation. It might be called dial-a-ride, small bus, connector service, or other different names. Explain what you need, and ask not only if they provide it but if there are other services they know of if it's not a function of their agencies' services.

You could also try contacting Borgess (Grand Rapids) and Bronson (Portage) Hospitals to find out if they operate services or if they know of any. These are major hospital systems in those areas.

Also, research online to find point to point transportation services in those areas. Try searching under "ambulance", "transportation for disabled", etc.

And as FF suggests, the AAA for western Michigan probably has lists of companies providing point to point transportation.

You would need to contact the patient's primary doctor to see is he/she has any information for such a program. Or contact your local council on aging https://www.agingcare.com/local/Area-Agency-on-Aging
