Double check if Medicaid is paying for the facility....get that figured out first.
While you are doing this, check out other facilities that you are interested in. Check them out without notice. Pop in and see how they are. Then talk with Medicaid and get a direct answer from them and steps that you need to do to transfer..

And contact the ombudsmen! And perhaps aps.

Crimson, have a long talk with mom's MD, because it may not be the nursing home, it may just be dementia is taking a bigger toll. Yes, they do lose weight, and bloodwork would say why.

Crimson, are you on site, and have sern the vermin? or is small room etc, being reported to you by mom?

When I transferred my father from an awful place, I contacted his surgeon to make sure I did whatever was bneeded to be done to ensure Medicare payment. Medicaid was not involved.

The nurse practitioner said they would write a new script for the secondary facility. I made arrangements there, then notified the existingfacility we were leaving. They made transport arrangements, but if I ever did it again I'd make them myself as the transport ambulance was affiliated with the joint venture which operated the rehab facility. The ambulance driver was unprofessional, asked pointedly why I was transferring my father (none of his business), lectured me, then sped all the way to the next facility. I was afraid I'd be ticketed when I tried to keep up with him, but I honestly didn't trust him enough to take the route I normally would have.

If you have a phone camera or digital camera, discreetly take photos, with a date, of the rodent activity. After you get your mother safely transferred, send copies of the photos to the local health department, ombudsperson, and any other potential supervisory and/or interventionary agency. You could also send them to local news stations - many have these problem solvers who enjoy fighting on behalf of others.

babaloo and Gardenartist have a good point, get pictures, because the rats and roaches sometimes are hallucination activity. Dementia stories are crazier than science fiction.

I do have pictures. I sent them to the local Health department but have not heard anything. Sunday I went to see her and she fit into a size 0. I have heard some of these stories. She called my brother 5 times tonight to tell him that I haven't seen her since January and they were raping her. When I called back they were trying to put her bracelet back on her so she wouldn't walk out again. APS was involved but they have since dropped her case because the original complaint has been handled...they made sure of that. I've complained to the main corporate and never heard back. Then the other day I received a bill for $50,000. I still haven't been able to wrap my head around her disease let alone everything else they claim. Its very irritating! The last time I talked to the business office was last month and we were tying up loose ends of the Medicaid. She told me that they were going to release her to a cot in my living room if the Medicaid didn't go through. Now it's in place and they are charging me? I think I'm missing something somewhere! I hate having her there and I'm worried about her.
