Thx in advance especially during Olympics when we can watch humans at their physical peak! My dad is 87, stage 5 Parkinson's and ckd and a gazillion other things. Finally got him into a snf ....he's now hallucinating, can't stand up, etc. There's also no $ left. My dad needs to stay there because discharge is an absurd idea....I've said unsafe discharge 1000x. I was given notice that Medicare would stop paying because he has "platued." And in 48 hours we'd need to come up with a $13k deposit. That can't happen and he can't leave. We have given them almost everything needed for Medicaid except for the social security cards .... because guess what? People who have loved for 80 plus years lose crap. We can't get into the social security office until march 17. Do I just keep appealing? Are they just posturing? There's no $. He's in disasterous shape. What am I supposed to do? Not to mention....this country is broken.
My interpretation of your post is your father has maxed out all the SNF rehab 20 days and Medicare is only going to pay for two more days at 100%. You now need to transfer him to Long Term Care (Rehab will not continue). Long Term Care is different than SNF.
SNF is for rehab and up to 20 days is 100% covered by Medicare. After 20 days the insurance will decide if he is non progressing. If deemed non progressing you can file an appeal (the facilities business office will be able to guide you through this). Appeals are hard to will but not impossible and you should always apply for an appeal. If the appeal is awarded your father will go under additional review for more rehab every 10 days. You will need to refile another appeal and there is a copay responsibility after 20 days regardless if the appeal is successful. The patient has a 100 day maximum to fully recover and be discharged or will need to transfer to Long Term care. During the appeal the patient cannot be booted out but if not awarded the patient is responsible for the paying the coinsurance.
If the patient transfers to Long Term Care the patient will be billed the self pay rate the facility has set. I am assuming that is the 13K your post stated. If the patient can not pay the self-pay rate they will need to apply for Medicaid long term care. The facility will need to have an open bed Medicaid bed for the patient to transfer to or you will need to find another facility.
If your father can pay the $13k, there is an open Medicaid bed and you like the facility it is wise pay the bill. It is your right to find another facility but you will be billed for the days your father is in a Long Term bed. If you need to transfer him to another facility the social worker can help guide you to other facilities with open beds. Most social workers have a network relationship with other facilities social workers. However, it is up to you to do the transfer and work with the other facilities application process.
Immediately start the Medicaid Long Term application process. I always advise to retain an eldercare attorney to guide you through the long and tedious process.
Medicare will not pay if Dad is not making progress. Only the first 20 days is paid 100٪. 21 to 100 is 50% and the other 50% comes out of Dads pocket or his supplimental pays the difference. That did not happen with my Mom, she had to pay $150 a day. If she had been in the whole 80 days, she would have owed 12k.
If this facility has no longterm care then it will be up to you to find a place for him.
PS, its not a scare tactic. Medicare only pays if the person is making progress.
